Is Your Skincare Working, or Just Looking Like It Is?

Why you can't always tell the difference, and the standard we built to settle it.


Your skin looks better after using a product. But better how?

It's a harder question than it sounds. You can see that your skin looks smoother, feels softer, sits a little plumper than it did last week. What you can't see is whether the formula changed anything beneath the surface, or simply sat on top making things look good for the day.

This isn't a beginner's problem. It's one even dermatologists run into. "Even as a dermatologist, when I read a clinical study based mainly on visual grading or self-reported improvement,I often can't tell you whether a product is doing something beyond simply moisturising the skin," says Dr Clare Kiely, our chief medical officer. "I see that same confusion in my patients every day."

If a trained eye reading the actual data can't always separate real change from a moisturising effect, a label promising "clinically proven" results gives you very little to go on. So let's go deeper, starting with what "working" would even mean.

What's holding your skin up

Before you can judge whether something is working, it helps to know what it would be working on.

Your skin is held up by a scaffold. Collagen gives it strength and structure. Fibrillin and elastin give it bounce, and the ability to spring back. Over time, and with UV exposure, that scaffold is broken down faster than the skin can rebuild it. The lines, the loss of firmness, the slackness we tend to associate with ageing are largely the visible, downstream result of that structural change happening underneath.

"Skin is a working organ, and most of what determines how it ages is happening in layers you'll never see in the mirror," says Dr Clare. "The scaffold, the cells that maintain it, the barrier on top, they all work together. Healthy-looking skin tends to follow from that deeper structure being well supported, not the other way around."

Which is why a skincare product can change how your skin looks in two quite different ways.

The first works at the surface. Hydration plumps the outer layer, softens the look of fine lines, and leaves skin smoother and more comfortable. This is real, valuable, and worth having every day. But it works above the scaffold, and it eases off once you stop.

The second is structural: supporting the collagen and fibrillin underneath, the architecture that determines how skin holds up over years, not hours.

Where skincare works

Here's the catch. Early on, the two can look identical in the mirror, smoother, fresher, plumper, even though one fades when you stop and the other is quietly supporting the structure underneath. The surface result, genuinely good as it is, can't on its own tell you whether anything deeper has changed. For that, you have to look further down.

So how would you ever know?

This is the problem our co-founder, Professor Chris Griffiths OBE, has spent much of his career on. A dermatologist whose research is among the most cited in the field, he helped develop the biopsy methods that measure what skincare actually does to living skin, rather than what it appears to do on the surface.

"Skincare has a proof problem," he says. "'Clinically proven' can mean many different things and often it refers to a study that measured the appearance alone. We wanted to build a standard that connects cellular-level rigour all the way with visible real world outcomes.

That standard is Cell-to-Surface™. If appearance alone can't tell you whether a formula truly works, you have to test deeper, at a level the surface can't account for, the biology itself, and then confirm it carries through to what you can see and feel. So it asks three separate questions of every formula:

  • Cell: is the skin's underlying biology actually changing?
  • Surface: is that change measurable by instrument, against a control?
  • Mirror: do real users notice and feel it, under blinded conditions?

The rule is simple: a product tested to Cell-to-Surface™ carries no efficacy claim until the formula has cleared all three tiers. A good result in the mirror is welcome, but on its own it isn't enough to earn a claim. The three tiers have to agree.

The tier that settles the question

The first tier is the one that answers the question this article opened with, so it's worth spending a moment on.

Testing at the cell means measuring the biology directly rather than inferring it from how skin looks. Instead of asking "does this appear to work," you measure whether specific genes and structural proteins have actually responded, using either living human skin or reconstructed full-thickness skin models grown in a lab.

Our worked example here is the Manchester Patch Test Assay, a peer-reviewed biopsy method developed at the University of Manchester. It measures, on real human skin, whether a formula rebuilds fibrillin and collagen, the exact scaffold proteins skin loses to ageing and UV. That's a far harder and more meaningful question than whether skin looks better, because the proteins either increased or they didn't, and a biopsy can't be flattered by good lighting or a temporary plumping effect.

One important detail: this tests the finished formula, not a single ingredient in isolation. Which matters more than it might sound, because what's printed on an ingredient list and what a formula actually does on your skin are not the same thing. (Look out for a future post covering this).

The other two tiers confirm the change carries through. Surface uses instruments to quantify things like wrinkle volume, hydration and UV protection over time, measured against a control and tested for statistical significance. Mirror puts the formula in front of independent consumer panels, scored against a pass threshold the lab sets before the panel begins, so the bar can't be quietly moved to fit the result.

What it looks like when a formula passes

It's easier to see how this works with a real routine. Our two products, Age Defence and Night Repair Therapy, were each taken through all three tiers.

At the cell, Night Repair Therapy produced a two-fold increase in both collagen and fibrillin in the Manchester assay: structural proteins, measured directly, not inferred from appearance. On the surface, the same formula reduced wrinkle volume at 1, 4, and 12 weeks. In an independent consumer panel, 86% of users noticed reduced fine lines.

Age Defence went through the same three tiers, tested at the cell via gene-expression analysis at Skin Life Analytics, where it produced a 3.7x reduction in UV-induced collagen damage compared with unprotected skin. On the surface, its SPF 50 and PA++++ protection were confirmed by in-vivo testing. In its consumer panel, 88% of users reported healthier-looking and healthier-feeling skin.

The point of laying it out this way isn't the individual numbers. It's that each result was asked the same three questions, and the claim only stands because all three answered yes.

Why this changes how you choose

Knowing that a good week-one result can come from either depth changes what you ask of a brand. Not "does my skin look better," which a lot of things can achieve for a day, but "is this supporting the structure that determines how my skin holds up over the long term, and how do you know?"

That's the real benefit of a standard like this. Not that we test rigorously for its own sake, but that you can stop guessing whether your effort and your money are buying you something real. We'd rather support your skin's long-term health honestly than chase a result that looks good for a week and fades.
